Enrollment Paperwork
In order to enroll your child(ren), you will need to fill out our general Enrollment Packet, read the Parent Handbook, and fill out the corresponding addendum paperwork below.
Physical Forms completed by a medical professional and Immunization Records also required for the enrollment of ALL children. Some paperwork must be signed in person. *Turning in an enrollment packet does not guarantee an immediate slot.
